This E-Sign Consent & Disclosure ("Disclosure") applies to your use of the online payment portal (the "Portal") operated by Slovin & Council, Co., LPA ("we," "us," or "the Firm"). The federal Electronic Signatures in Global and National Commerce Act (the "E-Sign Act") allows the use of electronic records and electronic signatures to satisfy any law that requires information to be provided or signed in writing. By agreeing to this Disclosure, you consent to receive records and communications electronically and to use electronic signatures in connection with your current and future account(s).
1. Scope of Consent
By agreeing to these terms, you consent to receive electronic notifications, disclosures, agreements, statements, and documents related to your account(s) and any payment agreement, as well as any broader categories of information as specified. This consent applies to all relevant transactions and communications with the Firm unless stated otherwise. We may provide these communications to you through the Portal or by email or SMS.
2. Paper Delivery and Copy Request Rights
You have the right to receive any required document associated with your account or payment agreement in a non-electronic (paper) form. You may request a paper copy of any record at no charge by contacting us using the information below. Should you choose to withdraw consent for electronic documents, please be aware that this may cancel your payment plan or limit your ability to use the Portal.
3. Withdrawal of Consent and Contact Information Updates
You may withdraw your consent to receive documents electronically at any time by contacting us using the information below. Withdrawal of consent is effective only after we have had a reasonable opportunity to act on it. You are responsible for promptly notifying us of any change to your email address, mailing address, or other contact information so that you continue to receive communications from us.
4. Hardware and Software Requirements
To access and retain electronic documents, you will need:
• A device (computer, tablet, or smartphone) capable of receiving, accessing, displaying, and retaining electronic records.
• An active internet connection.
• A current version of a web browser (for example, Chrome, Firefox, Safari, or Edge).
• A valid, active email address.
• Software capable of viewing and printing PDF documents.
• Sufficient storage space to retain records, or a printer to produce paper copies.
5. Affirmative Consent
Your electronic consent signifies that you are able to access and view electronic documents in the format provided. By providing your consent electronically through the Portal, you confirm that you have the necessary hardware and software described above and that you are able to receive and retain electronic documents. We will maintain a record of your consent for compliance purposes.
6. After-Consent Disclosure
If there are changes to the hardware or software requirements necessary to access or retain electronic documents, we will notify you through the Portal or by email. You have the right to withdraw your consent at any time. If any change materially impacts your ability to access documents, we will provide updated information and, where required, obtain your affirmative consent again.
